To help members maintain their independence and vitality, while living as an active part of the community. To support members, and enable them to continue living in their homes, we provide health, social, and nutritional intervention; an organized program of nursing services; supervision; maintenance; therapy; and socialization.<\/p>\n<p>Working with a member\u2019s family and physician(s), The Mentor Network staff develops a comprehensive plan of care that best meets a member\u2019s individual needs. Each Adult Day Health center has a qualified team of staff including a Program Director, an Assistant Director, Nurses, Social Workers, Activities Leaders, and Program Assistants.
